You will receive a letter giving you instructions, a summary of the procedures for processing CEU requests, and the Request for Approval Form CS/02.
The AVC office will process all information for your program, sending to NC State's Registration and Records the registration list containing all required information on your program participants. This information becomes a part of every participant's permanent record on a CEU transcript at NC State University. Records of your CEU program will be filed in the office of the AVC.
The office of the AVC will bill you $6.00 for each registrant in your CEU program. There is a $25.00 minimum fee if you have less than five participants. Number of registrants is determined by the roster that you send to the AVC office.
Requests for a transcript must be made in writing by individual program participants to the Office of the Registrar, North Carolina State University, Box 7313, Raleigh, NC 27695-7313. The request must include the participants date of birth and NC State Personal ID number. The cost for a transcript is $10.00 made payable to NC State University.
