CEU Approval Form Procedures

North Carolina State University is certified to award CEU's by the International Association for Continuing Education and Training Conference (IACET)

Initiating Contact

Please mail the completed CEU CS-02 Request for Approval to Award CEU Credit Form ( MS Word or pdf) to the Office of the Assistant Vice Chancellor for McKimmon Center for Extension & Continuing Education, Box 7401, North Carolina State University, Raleigh, NC 27695-7401 with a copy of pertinent activity materials and program/instructional outline.

The Request for Approval to Award CEU Credit Form CS/02 is due at least 20 business days prior to the scheduled date of the activity. This information will be reviewed for approval to ensure compliance with the International Association for Continuing Education and Training (IACET). The information below is a guide for completing the application form.

Request for Approval to Award CEU Credit Form CS/02

  1. API number is assigned by the office of the AVC.
  2. The Activity Title must be as descriptive as possible.
  3. Program - choose new or repeat. New means the activity is being offered for the first time. Repeat means the activity has been previously approved and offered during the current or previous year.
  4. Number of days Activity-add up the number of days of the activity.
  5. Number of instructional hours means number of contact hours as defined in the IACET criteria.
  6. The number of CEUs to be offered will be assigned by the Office of the AVC.
  7. Needs assessment for the target audience should be identified by a planning committee, a study, or the appropriate technical professional. It is not necessary for every activity conducted during the CEU program to have a separate needs assessment, but the rationale and planning for each activity should be the result of needs that have been identified by some assessment method.
  8. NC State University faculty/extension content specialists are individuals having activity content expertise who have reviewed and documented the program content as being acceptable by the University.
  9. Program Sponsor means the name of the organization or NC State University unit that is offering the course or program.
  10. If the program sponsor is an NC State department/unit, the OUC number must be entered. This is necessary for University Planning and Research to satisfy University of North Carolina General Administration reporting requirements.
  11. Co-sponsor, when appropriate, is the name of the organization that may be in partnership with the program sponsor.
  12. If the program sponsor is not an NC State unit, the department of the content specialist identified in #8 above will be noted as the co-sponsor. The OUC number of the department will be used for reasons described in #10. If the program sponsor and co-sponsor are both NC State units, only the program sponsors OUC must be recorded to prevent double counting on reporting requirements.
  13. Who should attend? - Identify the target audience by groups.
  14. Number of participants expected means number expected to attend program.
  15. Person certifying participants can be the instructor or another individual who has on-site knowledge of participation of registrants.
  16. The name of the primary instructor should appear on the form.* Attach a list of additional instructors and resumes for all instructors or presenters.
  17. Program Director is the name of the individual who is responsible for the administration and logistical aspects of the program.
  18. Evaluation refers to the measurement of the quality of the activity as a whole, and assessment refers to the measurement of individual learning outcomes. Every activity must identify the means by which it will be evaluated. Assessment procedures are optional but preferred. Identify the test when the activity includes some kind of testing technique.
  19. CIP Content Code ( pick list). Choose the category that best identifies the content area of the program.
  20. Industry Served Code ( pick list). Choose the industry most served.
  21. Impact Code ( pick list). Choose the category that best identifies the impact objectives of the activity.
  22. College/division-fill in the college/division of the program if applicable.
  23. Briefly record the learning outcomes (i.e., behavioral or performance objectives such as skills, knowledge and/or attitudes the learner should be able to demonstrate during or following the learning experience).
  24. Activity description should briefly describe each topic being offered. Please do not put “see attached” on the approval form, you must provide language describing activity. However, the program and/or instructional outline can be attached.*
  25. Activity Site. City, County, State, and Country.
  26. Registrants Locale ( pick list). Reflects the residence of the majority of participants served.
  27. Dates of Activity. Give the beginning and ending dates of the program.

Authorized Signature via Memo of Understanding is only required if the organization has a pre-signed Memorandum of Agreement with the office of the, AVC.

* Information recorded on the Approval Form will be entered into an electronic database. Therefore, please do not use terms such as "see attached material" in lieu of completing the form.

Activity Registration Form

The program registration form is for the use of the program coordinator. It is not necessary to send this form in with other required documents. The program registration form should include registrant's full name, address, assigned NC State Personal ID number; date of birth; telephone number; activity title and API/program number; activity provider's name and address; location and completion date of the activity; and number of CEUs awarded.

Registration List

All program information must be submitted for processing at one time with all of the required registrant information. Late registrations cannot be processed because CEUs cannot retroactively be awarded after the course has been filed with the Registrar. Individual participants cannot select whether or not they will receive CEUs; satisfactory completion of the stated criteria as developed and evaluated by the instructor/sponsor is the sole factor. All participants satisfactorily meeting the program criteria should be on the roster. An NC State ID number will be assigned to each participant.

Submit a typed alphabetical registration list giving activity title, location, date of activity, API/program number, and number of CEUs awarded as your heading. List every participant's name, address, date of birth and NC State ID number (if known). This list must be signed by the qualified person certifying attendees met the minimum requirements for satisfactorily completing the program. All program information must be submitted for processing within 30 days after program completion with all of the required registrant information.

Transcript

Request for a transcript must be made in writing to the Office of the Registrar, North Carolina State University, Box 7313, Raleigh, NC 27695-7313. This request must include the participant's date of birth and NC State-assigned ID number and a $10.00 check or money order payable to North Carolina State University. Please instruct the participants not to request a transcript until 60 days after the program is completed.
